The term Telehealth is defined as the use of Telehealth technologies to provide clinical care when the provider and the patient are in different locations. Telehealth in the VA takes place in 3 ways: (1) Home Telehealth (HT), (2) Clinical Video Telehealth (CVT) and (3) Store-and-Forward Telehealth (SFT).

Home Telehealth (HT) is defined as a program into which the enrolled Veterans physical and/or mental health are monitored daily and managed by a care coordinator who works closely with your specific providers. The goal of HT is to reducing complications, hospitalizations, and clinic or emergency room visits for Veterans who have chronic health problems. HT program monitors Veterans with hypertension, diabetes, chronic heart failure, chronic obstructive pulmonary disease, depression, post traumatic stress disorder, and weight management.

Clinical Video Telehealth (CVT) is defined as the use of video conferencing to assess, treat and provide care to a patient remotely. Typically, CVT links the patient(s) at a CBOC to the provider(s) at VA Hospital. Clinics that are currently utilizing CVT are: Neurology, Occupation/Physical Therapy, Oxygen Reevaluations, VISOR, Pharmacy, Audiology and Mental Health. Please note that not all clinics are available at all CBOC’s. Contact your clinic provider for availability or interest.

Store and Forward Telehealth (SFT) is defined as the use of technologies to acquire and store clinical information such as data and images that are then forwarded to a provider at another location for clinical evaluation. The goal of SFT is to provide screening, diagnostic or treatment services where time and/or distance separate the patient and provider. Current SFT programs are Optometry in the screening for diabetic retinopathy and Dermatology.
