The Medical Foster Home program is a safe alternative option to assisted living or nursing home care. The Veteran resides with a carefuly chosen caregiver and their family in their home. MFHs provide a friendly and homelike atmosphere where Veterans have freedom, independence and privacy. Caregivers are thoroughly screened and monitored on a continual basis.
To qualify for the Medical Foster Home program, a veteran must:
be diagnosed with a chronic disease or terminal illness
have a disabling condition, be dependent, or require assistance or supervision
unable to live independently
meet criteria for or already enrolled in Home Based Primary Care (HBPC)
have minimal family or community support and/or family is unable to provide 24-hour care
want to move from current home, assisted living or nursing home facility and is willing to live with non-family
have financial resources to pay caregiver

The Medical Foster Home Program provides Veterans:
An alternative option to nursing home placement and long-term care.
Primary care in the home in partnership with the VA Home Based Primary Care.
Alternative for dependent, chronically or terminally ill Veterans with limited family support.
Community-based living arrangement for adults who cannot live independently due to physical or mental disabilities.
Room and board, personal care, meals, protection and one-on-one attention.
Signed contract between MFH caregiver and the Veteran, family or legal representative.
