Lebanon VA Medical Center

Community Care

The Office of Community Care (OCC) provides compressive case management for Veterans who are eligible for medical care in the community through the MISSION Act. OCC manages inpatient and outpatient care in the community and assists the Veteran with authorizations, coordination and case management of care, and discharge planning.

Veterans must receive approval from the VA before receiving care from a community provider to avoid being billed for the care, except for urgent and emergent care. After discussing community care eligibility with your primary care provider and a consult has been placed, if you have additional questions please contact the appropriate community care team:

Urgent Care Benefit
Veterans are eligible for the urgent care benefit if they are enrolled in VA health care and have received care through the VA within 24 months prior to receiving urgent care. To find an urgent care provider in VA’s contracted network of community provider’s go to or call 1-844-698-2311 or 1-877-881-7618 .

Emergency Care
During a medical emergency, Veterans should immediately seek care at the nearest medical facility. A medical emergency is an injury, illness, or symptom so severe that without immediate treatment, you believe your life or health is in danger.

Veterans do not need to check with VA before calling for an ambulance or going to an emergency department. It is, however, important to promptly notify VA after receiving emergency care at a community ED. Notification should be made within 72 hours of presenting to a community emergency department by calling 1-844-724-7842.
